Appointment Scheduling

Patients are seen by appointment only. If you are having a problem, which requires immediate attention, we will schedule you accordingly. For your convenience, we have later afternoon hours on Monday and Wednesday.

** A 24-hour cancellation notice prior to a scheduled appointment would be appreciated.

Insurance Billing

We accept most insurance companies and will attempt to bill all claims, but you will be responsible for all co-pays, deductibles, and non-covered services. Payment plans can be arranged by calling our office.

** If you have an HMO insurance, you may need to call your primary care physician for a referral prior to your appointment.

Prescription Refills

For all refills, call during office hours or submit your refill request via the Patient Services-Email Requests area of our website.

Nurse Call In (Sick Calls)

For your convenience, we have nurse call-in times Monday thru Friday 8-9 and 11:30-12:30. You should use this for all sick calls and injection questions. Calls regarding medical emergencies will be taken anytime during normal office hours listed below. Non-emergency questions can be submitted online via the Patient Services-Email Requests area of our website.

Emergency Calls (After Hours)

For all after hours medical emergencies, please call the main office number and leave a message for the on-call physician.  Your call will be returned as soon as possible.  All other calls should be referred to normal office hours.
